看了大家讨论了这么多，我只好查一查资料，以免误导大家

结果如下：

Direct Cause: 直接原因

The cause that directly resulted in an event
(The first cause in the chain
The physical link to the event. The answer to the event question.
直接导致事件发生的原因(原因链中的第一个原因)，
直接跟事件有关，是事件的答案

Contributing Cause: 间接原因
Causes that contributed to an event but, by themselves, would not have caused the event. (The causes after the direct cause)
是导致事件发生的原因，但它自己不能单独使事件发生
（直接原因之后的原因）

Root Cause: 根本原因
The Fundamental reason for the event
(The last cause in the chain)
事件的基本原因（原因链中的最后的原因）

Event: Didn’t get to work on time. 事件：上班迟到了
Why were you late? 为什么迟到？
1.Car wouldn’t start. 汽车没启动
2.Why didn’t it start? 为什么汽车没启动？
3.Battery was dead. 电瓶没电了
4.Why was the battery dead? 为什么电瓶没电了
5.Dome light stayed on all night. 汽车顶灯亮了一夜
6.Why was the light on? 为什么灯一直亮了一夜？
7.Kids played in car , left the door ajar. 孩子在车里玩，车门没关上

One, two, or more, depending on how many problem statements you have.
Each problem will have it’s own cause chain ending in a root cause.

一个，两个或更多，是基于你有多少个问题的描述，任何一个都将会通过各自的原因分析链，发现一个根本原因。


Sometimes the event as reported or as found, is not indicitive of the true problem.

What is the real problem?

Is there more than one problem?

Understanding the problem is the 1st key to this process.

有的报告或发现的问题并不是真正的问题，

什么是真正的问题？

是多个问题吗？

理解什么是问题是查找原因的关键的一步。

这些又是很多同仁深恶痛绝的一线企业的工作方式和流程呀，但既然探讨Root Cause,我个人认为不把这些讨厌的条条框框搬上来，还是说不清楚的。
