最近看下8D一些案例,反先有些要用Q币购买的案例,实际上写的很不怎么样.

ON PULSE P/N FET3986 Rev: A
D1: USE TEAM APPROACH

D2: DESCRIBE THE PROBLEM
Customer MGE reported terminal connector wire reversed issue found at customer production line. Only one failure found with D/C 0420. 1pc with date code 0420 was returned from MGE to Pulse for analysis.
The returned units were performed electrically test & evaluation. For detail findings, please refer to below table, base on below findings, the reported issue was confirmed in Pulse.
S/N Date code E-test result. Visual inspection result
#1 0420-C T&R failed, and passed after rotate the sample 180’. PASS
D3: IMPLEMENT AND VERIFY CONTAINMENT ACTIONS
• Take all of inventory parts with quantity 290pcs to re-check.
Result: No failure found.
Responsible: SQ Fang /Flora Chan
Completed Date: 01/27/05
D4: DEFINE ROOT CAUSE:
During assembling two cables, operator wrongly put the end-wire of coil into #1 hole, and put the start-wire of coil into #3 hole. MI had clearly indicated how to put the two cables in the holes, so this is a human error.
And no clearing instruction on the test fixture to tell the operator how to test the parts, base on the electrical test result on D2, this product was tested using the wrongly testing method.
D5/D6: CHOOSE AND VERIFY PERMANENT CORRECTIVE ACTION/
To further improve our product quality and prevent the similar failure from happening again, we will implement the following corrective action:
• Send the photos of failed sample to production line for training.

Effective datee: 02/07/05
• Re-adjust the test fixture to assembly a stopping to avoid wrongly testing method with rotate the parts 180 degree using on production line, please see the pictures of fixtures as below:

• Do the same actions for the similar products.
