8D report中 Occurrence和Escape两项,存疑问,谁能帮忙?
请帮忙看一下8D report里面的一个东东。
这个8D report里每一步都有: Occurrence和Escape两项;我搞不清楚这两项具体是填什么。
STEP 4: Identify Root Cause: (attach supporting evidence)
The impact is caused during our loading process. After a hard impacted, the product was deformation, and it could not be normal used.
Occurrence:

The root cause has not yet been defined. This all pertains to steps 4 - 7 on the 8D. I would like to for you to complete a “5 why’s” sheet.
For example (these are just my words and may not be the same as your analysis)
Why did the muffler get impacted during the loading process?
Answer: It was dropped.
Why was it dropped?
Answer: The muffler was not secured or tied effectively to the forklift truck.
Why was it not tied to the forklift?
Answer: The operator did not think it was needed.
Why didn’t the operator think it was necessary?
Answer: There were no instructions for the operator to tie the muffler to the forklift.
Why were there no instructions for this?
Answer: This part of the process was not defined in the process flow.
Thus, root cause in this case is “no instructions available to the operator to tie the muffler to the forklift to prevent it from being dropped and impacted”.
This instruction must include how to tie it onto the forklift and then training must be done to assure the operator knows how to do it. Lastly, the training must result in effective operator work so this issue will not reoccur. Again, this root cause analysis all pertains to the “Occurrence” portions of steps 4 – 7 on the 8D.
All references to the inspection of the muffler should be part of the “escape” portions of the 8D.
